… inefficient and an eyesore but I was thinking (brain hurts) about them. Do they work by funnelling air from outside? As in poisoned air, now full on in winter, pumped in to sicken residents? Was this planned?

No! In the installation shown the heat transfer is done through the insulated pipes you’ll see going from the external unit to the house. This would either be in the form of refrigerant gas or water. There are also air-to-air units, but these still have a heat exchanger, so there’s no direct transfer of outside air to inside.
